How to Know if You Qualify for ODSP Special Diet or Medical Forms
If you’re receiving support through the Ontario Disability Support Program (ODSP), you may be eligible for additional benefits — including a Special Diet Allowance, medical forms for disability claims, and other health-related supports.

What is the ODSP Special Diet Allowance?
It’s a financial benefit provided by ODSP for individuals who have health conditions that require special diets or food restrictions.
Common qualifying conditions include:
• Diabetes
• Celiac disease
• IBS or IBD (Crohn’s, Ulcerative Colitis)
• Lactose intolerance
• Hypertension
• Chronic constipation
• Pregnancy
• Low iron/iron deficiency anemia
If your condition is eligible, a healthcare provider (like a Nurse Practitioner) must complete and sign the ODSP Special Diet Form.

We can. At Pure Health Primary Care, our Nurse Practitioner is fully licensed to assess, diagnose, and complete:
• Special Diet Forms
• Pregnancy Confirmation
• ODSP Medical Reports
• Disability Determination Packages (DDP)

How Much Does It Cost?
• Form visit (non-members): $100
• Follow-up or additional forms: $80
• Members: Included in your 8 yearly visits ($400/year)
We’ll also provide a formal invoice you can submit to your caseworker if needed.
