Membership Plan
PureHealth Membership is an optional program for patients who value continuity, convenience, and enhanced clinic support.
Membership includes:
- Enhanced scheduling support, with most appointments available within the same week and often next day when possible
- Ongoing follow-up coordination and clinic support
- Secure communication with the clinic
- Preventive care planning and personalized health guidance
- Administrative support for forms, documentation, and care navigation
- Additional time and support for more complex care needs when appropriate
Annual Membership: $550/year
Annual Membership (Ages 50+): $699/year
Membership is optional and supports non-insured services, administrative support, and enhanced clinic features.
Pay-As-You-Go Services (Non-Members)
Flexible access for patients who prefer to use non-insured and administrative services as needed.
Non-Insured & Administrative Services: $49
$49/service on weekdays. $69/service on weekends. $97 per service for ages 50+.
Virtual Care Services
Administrative Requests & Non-Insured Virtual Services: $49/service
Prescription Renewal: $40/request
For existing medications and requests that do not require a full assessment.
Specialized Programs
Weight Management Program:
- Initial Assessment: $100
- Follow-Up Visits: $75/visit
Smoking Cessation Program:
- Initial Consultation: $100
- Follow-Up Visits: $75/visit
Travel Services
- Travel Consultation: $75/person
- Travel Vaccines: $75/vaccine per dose
Vaccine availability and costs vary. Some vaccines may be partially or fully covered by private insurance.
Form Completion
- Sick Notes: $45
- Disability Tax Credit Certificate (Form T2201): $150
- Insurance Forms (e.g., Short-Term Disability): $220
- Driver’s Medical Examination Form: $150
- Pre-Employment Medical Forms: $150
Other Services
- Cryotherapy for Skin Lesions: First lesion $75
- Additional Lesions: $15 each
- Ear Syringing: $40/visit
- TB Mantoux Testing: $89/step (2 visits)
